Overview
Thyroidectomy is a common operation in general surgery, but traditional open thyroidectomy will leave scars on the neck and bring about aesthetic impact. Therefore, thyroidectomy has developed several minimally invasive procedures, such as minimally invasive video-assisted thyroidectomy, transaxillary thyroidectomy, and natural orifice surgery.
Transoral endoscopic thyroidectomy vestibular approach (TOETVA) is a natural orifice surgery and has become one of the minimally invasive procedures for achieving “scar-free in neck” in thyroidectomy.
With the efforts of Dr. Jui-Yu Chen and colleagues, the Thyroid Center of Division of General Surgery, Taipei Veterans General Hospital, has actively developed "Transoral endoscopic thyroidectomy" In more than 300 cases, Taiwan's first "3D-Printed Patient Specific Safety Device" was used in the operation to achieve zero mental nerve injury. This device won the 2020 Taipei Veterans General Hospital Physician Innovation Award and the 2021 Symbol of National Quality. Academically, the research results of 3D-Printed Patient Specific Safety Device were published in the World Journal of Surgery in 2020.

Procedure
Patient preparation was the same as for standard open surgery, including neck extension and intubation, with a nasotracheal tube attached to a nerve monitor device.
Disinfect oral and facial surgical areas.
- Incisions and port insertion

Notifications
Thyroid surgery has several potential complications, including
- Bleeding that may sometimes obstruct the airway.
- Low levels of parathyroid hormone (hypoparathyroidism)
- Permanent hoarse or weak voice due to damage to the nerves of the vocal cords
