Computer-aided orthognathic surgery
Overview
Face is the unique identity of the individuals. The disproportional facial skeletons result in facial deformities and skeletal malocclusion. It may present with malaligned dentition, prominent / retrusive jaws, or facial asymmetry. Not only the chewing function will be affected, the facial aesthetics, phonation, and even the breathing problem are all related to the facial anomaly. Orthognathic surgery (OGS) is a fascinating solution to rebuild the proper jaw relationship. Hence, to achieve a well aligned occlusion and harmonious facial profile.
Features Summary
Orthognathic surgery is an operation to normalize the disproportional facial skeletons to improve the occlusal function and facial esthetics. It composes of maxillary and mandibular osteotomy procedures to reposition and fixation of the facial skeletons. The purpose of OGS is to achieve a functional occlusion and harmonious facial profiles. Facial bone contouring procedures could be added on to optimize the esthetic results. Currently, in Taipei Veterans General Hospital, computer-aided approach is routinely used in presurgical planning. In combination with the computed tomography images, digital dental model and the 3D photo from face scanning, a composite 3D virtual facial model is used for surgical planning. Based on the virtual planning, the 3D printed surgical guides are used to transfer the surgical plan and optimize the results by surgical navigation intraoperatively.
Presurgical phase:
1.Complete occlusion and facial analysis by orthodontists and maxillofacial surgeons.
2.Collect the data for virtual surgical planning, including the CT scans, digital dental models, with/without the 3D face photo.
3.Share decision making with comprehensive discussion on the treatment sequence, virtual surgical design, treatment goals, and possible complications.
Surgical phase:
1.Pre-surgical systemic survey and anesthesiologist consultation.
2.Operation with 3D printed guides and intraoperative navigation.
Postsurgical phase:
3.Wound care and diet / oral hygiene instructions.
Usually admission for 4 days.
Notification
Potential risks and complications included:
Self-limited swelling, nasal discharge/bleeding, lip numbness, trismus, joint pain...etc.
